Parents of Athletes,

Thank you for supporting Jemez Mountain School District athletics. Please use the attached form to register your student for the 2026–27 school year. You must create an account and upload a current physical for the upcoming year.

After creating your account, please ensure the following required NMAA documents are completed and submitted. Items indicate who is responsible: (Doctor), (Parent), or (Student).

• PPE Health History Form (2023 Version) — (Doctor)
  Good for 2026–27 academic year | Start Date: 3/31/26 | End Date: 5/30/27

• PPE Physical Examination Form — (Doctor)
  Good for one year, or for the entire next school year after April 1st | Start Date: 3/31/26 | End Date: 5/30/27

• PPE Medical Eligibility Form — (Doctor)
  Good for one year, or for the entire next school year after April 1st | Start Date: 3/31/26 | End Date: 5/30/27

• Consent to Treat Form — (Parent)
  Good for 2026–27 academic year | Start Date: 3/31/26 | End Date: 5/30/27

• NMAA Concussion in Sports form — (Student: must complete the Concussion course on their own)

• PHYSICALS MUST BE DATED ON or AFTER APRIL 1st to be VALID FOR 2026/27.
